Physiotherapist

Roles & Responsibilities:
• Provide high-quality physiotherapy services to HammondCare Support at Home clients including individual home visits and group activities
• Assess clients’ functional care needs and develop in-home rehabilitation programs to improve independence and quality of life
• Collaborate closely with interdisciplinary team members to set care goals and deliver optimal client outcomes
• Participate in quality improvement initiatives and continuing professional development
• Provide clinical supervision for students, where appropriate
• Uphold HammondCare’s mission, values, and person-centred care standards

Qualifications:
• Qualified Physiotherapist
• Current AHPRA registration
• Practical experience or sound knowledge of physiotherapy in rehabilitation or aged care settings
• Valid driver’s licence and access to a reliable vehicle

Experience:
• Experience in rehabilitation, aged care, or community physiotherapy preferred

Skills:
• Strong communication and interpersonal skills
• Ability to plan and manage time effectively
• Ability to work independently and collaboratively within a multidisciplinary team
• Commitment to person-centred, evidence-based care
