Role & Responsibilities

• Conduct post-discharge teleconsultation follow-up calls
• Assess recovery status and monitor symptom progression
• Identify early warning signs and escalate critical cases
• Evaluate medication adherence, labs, diet, physiotherapy compliance
• Provide evidence-based medical guidance (non-prescriptive)
• Guide patients on next-step care pathways and referrals
• Educate patients on lifestyle modification and self-care practices
• Maintain accurate clinical documentation in digital systems
• Collaborate with program managers to improve patient compliance and outcomes

Qualifications & Experience

• MBBS (Mandatory)
• Freshers / 1–2 years of clinical practice
• Family Medicine / Internal Medicine / Emergency / Community Health preferred

Skills

• Strong clinical judgment and analytical ability
• Excellent communication and patient counselling skills
• Good knowledge of post-hospitalization care protocols
• Teleconsultation and digital health platform familiarity preferred
