Physical Therapist Assistant
About the Role
ROLE & RESPONSIBILITIES:
• Provide physician-prescribed physical therapy under a care plan established by a licensed Physical Therapist (PT)
• Deliver in-home physical therapy services to help patients regain mobility and independence
• Improve or minimize residual physical disabilities and support optimal functional outcomes
• Participate with healthcare personnel in patient care planning
• Perform skilled procedures as ordered by physicians and according to the plan of care
• Consult with the supervising PT regarding treatment changes
• Educate patients and family members on home programs and activities of daily living
• Prepare medical records, progress notes, and update care plans in a timely manner
• Attend patient care conferences and participate in performance improvement activities
• Follow agency policies, procedures, and quality standards
QUALIFICATION:
• Graduate from a two-year college-level program approved by the American Physical Therapy Association
• Currently licensed in the state(s) of practice
EXPERIENCE:
• Minimum 2 years of experience preferred
• Ability to exercise initiative and independent judgment
SKILLS:
• Strong knowledge of physical therapy procedures and rehabilitation techniques
• Ability to work independently while maintaining clinical standards
• Excellent communication and patient education skills
• Strong documentation and organizational abilities
• Commitment to patient-centered care
• Acceptance of agency philosophy and goals

WORKING ENVIRONMENT:
• Work indoors in agency offices and patient homes
• Travel required to and from patient homes
• Supervised by Registered Physical Therapist / Director of Therapy, Home Health
• High-risk exposure role
PHYSICAL REQUIREMENTS:
• Ability to participate in physical activity for extended periods
• Capability to stand for long durations while performing clinical tasks
• Ability to perform heavy lifting, bending, and regular physical movements
