Pharmacist Senior RAPID

Roles & Responsibilities
• Provide best practice pharmaceutical care to Respiratory patients within the Sunshine Coast Hospital and Health Service
• Function as a non-rotational senior pharmacist within the Respiratory team
• Conduct comprehensive medication history interviews and clinical medication reviews
• Develop and implement individualised pharmaceutical care plans
• Exercise independent clinical judgement and provide expert advice to medical, nursing and allied health professionals
• Support the Pharmacist Advanced Specialty Services Team Leader in Medication Management initiatives
• Contribute to continuous quality improvement activities to enhance medication safety
• Provide guidance to subordinate staff on clinical work practices, dispensing, distribution, production and quality assurance
• Promote safe, effective and evidence-based medication use aligned with Health Service policies

Qualification
• Bachelor of Pharmacy (or equivalent)
• Current registration with Australian Health Practitioner Regulation Agency (AHPRA)

Experience
• Significant clinical pharmacy experience in a hospital setting
• Experience in Respiratory or specialty clinical services highly desirable
• Demonstrated experience in medication management and quality improvement initiatives

Skills
• Advanced clinical knowledge in respiratory pharmacotherapy
• Strong clinical judgement and decision-making skills
• Excellent communication and interdisciplinary collaboration abilities
• Leadership capability and ability to mentor junior staff
• Commitment to patient-centred care and medication safety
