Pharmacist

Roles & Responsibilities
• Review and process medication orders for appropriateness of therapy, dose, frequency, route, dosage form, allergies, interactions, and relevant lab values
• Verify patient demographic details such as age, gender, and weight during medication review
• Counsel patients on medication use, indications, side effects, and potential drug interactions
• Document clinical interventions, recommendations, and clarifications in the electronic Health Information System
• Maintain control, security, and accurate records of all medications, including controlled and narcotic substances, in compliance with laws and regulations
• Provide drug information and clinical support to healthcare providers on drug selection, dosing, monitoring, and adverse effects
• Supervise, guide, and monitor junior pharmacists, residents, trainees, and students
• Support healthcare providers on medication management and safe medication-use systems
• Participate in quality improvement and performance enhancement initiatives
• Adhere to all hospital policies, procedures, and professional standards
• Engage in continuous education, training, and professional development

Qualifications
• Master’s Degree in Pharmacy
• OR Pharm.D
• OR Bachelor’s Degree in Pharmacy

Experience
• Minimum 2 years of relevant experience with a Master’s degree
• OR minimum 4 years of relevant experience with Pharm.D or Bachelor’s degree

Skills
• Strong clinical pharmacy knowledge and medication review skills
• Patient counseling and communication skills
• Knowledge of drug interactions, dosing, and monitoring
• Familiarity with electronic health information systems
• Attention to detail and documentation accuracy
• Ability to supervise and mentor junior staff
• Teamwork and interdisciplinary collaboration
