About the Role

Roles & Responsibilities
• Conduct comprehensive medication reviews to identify potential drug interactions and ensure patient safety
• Advise physicians and healthcare professionals on appropriate medication selection and dosage based on patient-specific factors
• Provide medication counseling to patients, including usage instructions, side effects, precautions, and storage guidance
• Monitor patient outcomes after initiation of therapy and recommend adjustments to optimize efficacy and safety
• Maintain accurate medication-related documentation and support clinical decision-making

Qualification
• Doctor of Pharmacy (Pharm.D.) degree from a recognized/accredited institution

Experience
• Minimum 2 years of experience in a clinical pharmacy or hospital pharmacy setting preferred

Skills
• Strong clinical knowledge of pharmacotherapy and medication management
• Good patient counseling and communication skills
• Familiarity with Electronic Health Records (EHR) and pharmacy management systems
• Attention to detail and commitment to patient safety
• Ability to work collaboratively with multidisciplinary healthcare teams
