Pharmacist

Roles & Responsibilities
• Review and interpret prescription orders to ensure accuracy, appropriateness, and patient safety
• Provide expert medication counseling, including dosage instructions, potential side effects, and drug interactions
• Conduct comprehensive medication reviews to identify and resolve drug therapy problems
• Optimize therapeutic outcomes through evidence-based medication management
• Collaborate with physicians and other healthcare professionals to develop individualized care plans
• Monitor patient response to therapy and recommend adjustments when necessary
• Ensure compliance with regulatory standards and pharmacy best practices

Qualification
• Doctor of Pharmacy (Pharm.D.) from an accredited institution
• Valid and active pharmacist license as per local regulatory requirements

Experience
• Minimum 2 years of clinical pharmacy experience in hospital or retail pharmacy setting

Skills
• Strong clinical knowledge and therapeutic decision-making ability
• Medication Therapy Management (MTM) expertise preferred
• Board Certification in Pharmacy Specialties (BCPS) preferred
• Excellent patient counseling and communication skills
• Ability to identify and resolve drug-related problems
• Strong collaboration and interdisciplinary coordination skills
