About the Role
Roles and Responsibilities:
• Deliver high-quality Occupational Therapy services using a person-centred approach
• Conduct assessments and provide interventions in clients’ homes to support independent living
• Develop tailored care plans based on individual client needs
• Monitor and evaluate client progress and adjust interventions accordingly
• Collaborate with Case Managers, Allied Health Professionals, GPs and other healthcare providers
• Prepare comprehensive reports meeting standards of NDIS, DVA, Medicare and Home Care Package providers
• Build and maintain strong referral networks with community centres and healthcare professionals
• Reflect organisational values of Care, Respect, Integrity and Service Excellence in all interactions
Qualification:
• Bachelor’s degree in Occupational Therapy
• General registration with AHPRA
• Full working rights in Australia
• Valid driver’s licence
• Access to own vehicle with comprehensive car insurance
• Willingness to undergo National Criminal History Check
Experience:
• Experience delivering Occupational Therapy services in community settings
• Experience working with older adults and individuals with disability
• Experience working within a multidisciplinary team environment
• Ability to work autonomously in community-based practice

• Occupational Therapy assessment and intervention
