• Manage chronic and complex physical and mental health conditions.
• Develop and review care plans aligned with patient needs.
• Coordinate referrals to specialists and allied health professionals.
• Support veterans navigating DVA-related health processes.
• Maintain high-quality, compliant clinical documentation.
• Collaborate with internal clinical teams to optimise outcomes.
Qualification
• Current General Registration with AHPRA.
• Fellowship of the Royal Australian College of General Practitioners (FRACGP) or Fellowship of ACRRM.
Experience
• Minimum 5 years’ experience in General Practice.
