General Practitioner

Role & Responsibilities
• Conduct Medicare bulk-billed in-home health assessments for elderly and patients with disability.
• Identify, assess, and manage chronic disease and preventive health risks.
• Develop and contribute to Chronic Disease Management (CDM) Plans.
• Coordinate care with the patient’s usual GP, specialists, and allied health providers.
• Maintain accurate clinical documentation and ensure Medicare compliance.

Qualifications
• MBBS (or equivalent) with general registration with AHPRA.
• FRACGP (or equivalent) preferred.
• Valid Provider Number and Medicare eligibility.

Experience
• Experience in general practice, preventive health, or chronic disease management.
• Interest or experience in aged care and disability health preferred.

Skills
• Strong clinical assessment and decision-making skills.
• Excellent communication and patient-centred care approach.
• Comfortable working in a mobile, home-visit care model.
