About the Role
Role & Responsibilities
• Provide on-site GP care to residents in aged care facilities across Geelong and Ballarat
• Deliver relationship-based, patient-centred medical care to older Australians
• Conduct comprehensive assessments, chronic disease management, and care planning
• Work collaboratively within a multidisciplinary team including nurses, telehealth specialists, and administrative staff
• Maintain accurate clinical documentation using system-based reporting tools
• Focus on quality care with a manageable caseload of approximately 25 patients per day
Qualification
• Recognised medical degree eligible for General Practice in Australia
• Fellowship with RACGP or ACRRM is mandatory
• General or Specialist AHPRA registration with no conditions
Experience
• Suitable for experienced GPs and newly Fellowed GPs
• GPs nearing completion of RACGP or ACRRM Fellowship are encouraged to apply
• Minimum availability of 1 day per week
Skills
• Strong clinical skills in primary and aged care medicine
• Excellent communication and relationship-building abilities
• Comfortable working with clinical software and system-based reporting
• Ability to work independently within a structured care model
• Patient-focused, compassionate, and team-oriented approach
