Community Occupational Therapist & Physiotherapist-Aged Care
About the Role
Roles & Responsibilities:
• Deliver goal-directed, reablement-focused care to clients referred through My Aged Care
• Conduct home-based clinical assessments for older adults
• Attend community centres to work with and supervise allied health staff
• Develop, review, and implement care plans
• Assess and recommend home modifications and assistive equipment
• Collaborate with clients, families, Team Leaders, Allied Health Assistants, and PCAs
• Maintain accurate clinical documentation
Qualifications:
• Bachelor's degree in Physiotherapy or Occupational Therapy
• AHPRA registration
Experience:
• Demonstrated experience working with older adults, including dementia care
• Knowledge of STRC, Post Acute Care, Transitional Care, CHSP, and NDIS funding/referrals
Skills:
• Strong communication and interpersonal skills
• Excellent organisational and time management skills
• Ability to manage an independent caseload
• Ability to work collaboratively with multidisciplinary teams
