Job Description
Murrumbidgee Local Health District is seeking a Community Care Physiotherapist / Exercise Physiologist (Level 3) to join its team at the Griffith Community Health Centre. This permanent full-time position focuses on leading innovative clinical initiatives and delivering evidence-based rehabilitation programs for patients with chronic and life-limiting conditions across the district. Responsibilities
• Lead the development of virtual and community-based chronic care initiatives to improve clinical outcomes
• Identify and implement cost-effective, evidence-based models of care aligned with healthcare policies
• Provide expert physiotherapy or exercise physiology consultation to staff across chronic disease management programs
• Mentor and supervise multidisciplinary healthcare professionals managing clients with chronic conditions
• Maintain accurate clinical documentation and activity statistics in accordance with NSW Health directives and KPIs
• Promote culturally competent healthcare and support improved outcomes for Aboriginal communities through engagement initiatives
• Maintain ongoing professional development through education, supervision, and evidence-based practice
Required Qualification
• Bachelor’s degree or equivalent qualification in Physiotherapy or Exercise Physiology
Important Requirements
• Current registration with Australian Health Practitioner Regulation Agency (AHPRA) for Physiotherapists or eligibility for membership with Exercise and Sports Science Australia (ESSA)
• Advanced clinical knowledge of chronic conditions including respiratory disease, heart failure, diabetes, dementia, and progressive degenerative disorders
• Experience managing rehabilitation and structured exercise programs
• Current NSW Working With Children Check
• National Police Check clearance
• Class C Driver’s Licence
• Vaccination compliance (Category A)
