Clinical Pharmacologist

Roles & Responsibilities
• Review and monitor prescriptions to prevent medication errors, drug interactions, and inappropriate dosing
• Identify, document, and report Adverse Drug Reactions (ADRs) and near-miss events
• Conduct medication audits and recommend corrective and preventive actions
• Support the Pharmacy & Therapeutics (P&T) Committee in formulary selection and review
• Evaluate drug efficacy, safety, and cost-effectiveness
• Provide therapeutic guidance to clinicians and nursing staff
• Assist in dose adjustments for special populations (pediatric, geriatric, renal, hepatic patients)
• Collaborate on Antimicrobial Stewardship (AMS) initiatives to promote rational antibiotic use
• Participate in Infection Control Committee activities as per NABH 6th Edition
• Develop SOPs for medication management and handling of high-alert medications
• Conduct training sessions on medication safety, ADR reporting, and rational drug use
• Support implementation of NABH standards under MOM (Management of Medication) and PSQ (Patient Safety & Quality Improvement)
• Ensure proper documentation and compliance readiness for accreditation audits

Qualification
• Pharm D (Doctor of Pharmacy)

Experience
• Minimum 4–5 years of relevant hospital experience

Skills
• Strong clinical pharmacology and medication safety knowledge
• Understanding of NABH accreditation standards
• Experience in antimicrobial stewardship programs
• Strong analytical and documentation skills
• Excellent communication and interdisciplinary coordination abilities
