Clinical Pharmacist
About the Role
Roles & Responsibilities:
• Provide clinical pharmacy services across various hospital departments
• Review and optimize medication therapy to improve patient outcomes
• Participate in ward rounds and collaborate with multidisciplinary teams
• Deliver services in General Medicine, Surgical, Paediatrics, Rehabilitation, GEM (including GEM@Home), Mental Health, Emergency Department, ICU, Oncology (Aseptic & Cytotoxic manufacturing), and Antimicrobial Stewardship
• Ensure medication safety and provide drug information support
• Supervise and educate intern pharmacists and undergraduate students
• Contribute to quality improvement and medication management initiatives
Qualification:
• Bachelor’s or Master’s degree in Pharmacy
• Registered Pharmacist with AHPRA
Experience:
• Hospital experience desirable but not essential
• Grade 1, Grade 2, or Grade 3 position depending on experience
Skills:
• Strong clinical and medication management knowledge
• Effective communication and teamwork skills
• Commitment to patient safety and quality care
• Teaching and mentoring ability
• Good organizational and documentation skills
