Job Description

Mater Health Services Southeast Queensland is expanding its Clinical Services team and is seeking motivated Pharmacists to join on a casual basis. The role operates across multiple hospital campuses located in South Brisbane, Springfield, and Redland.
This rotational position includes working in pharmacy dispensaries and inpatient wards across both public and private hospital settings. The pharmacist will collaborate with healthcare professionals, patients, and carers to provide comprehensive clinical pharmacy services.
Responsibilities include medication supply, clinical review, medication reconciliation, discharge planning, and patient education. The position supports a unified inter-professional team delivering care to over 500,000 patients annually, with further growth anticipated due to the expansion of Springfield hospital services.
Shifts are available across weekdays, evenings, weekends, public holidays, and potentially on-call rosters due to extended hours service at the Central Dispensary.

Responsibilities

• Manage supply of inpatient and discharge medications
• Review and check prepared medications before ward distribution
• Compile and document best possible medication histories for inpatients
• Perform medication reconciliation and clinical review of prescriptions
• Prepare discharge medications and discharge medication summaries
• Provide medication counselling and education to patients and carers
• Deliver medication safety advice and information to healthcare professionals
• Liaise with community providers and external facilities to ensure continuity of care
• Collaborate with pharmacists, technicians, and students while providing supervision when required
• Participate in quality improvement and service enhancement initiatives
• Support medication distribution to day units, procedural areas, Hospital in the Home, and infusion clinics
• Maintain professional standards aligned with healthcare policies and regulatory requirements

Required Qualifications

Educational Requirements:

• Bachelor of Pharmacy
• Current registration with AHPRA (without restrictions)
