Roles & Responsibilities:
• Provide clinical pharmacy services in hospital setting
• Review prescriptions and ensure safe, effective medication use
• Monitor patient therapy and report drug-related issues

• Maintain proper documentation and medication records

Qualification:
• Pharm D (Doctor of Pharmacy)
• Master of Pharmacy (M.Pharm) in Pharmacy Practice / Clinical Pharmacy

Experience:
• Minimum 1–2 years of experience in a hospital setting (Preferred)

Skills:
• Strong clinical knowledge and patient counseling skills
• Good communication and coordination abilities
• Knowledge of hospital pharmacy protocols
• Attention to detail and documentation accuracy
