Clinical Pharmacist Internship
About the Role
Roles & Responsibilities:
• Assist in clinical pharmacology and pharmacovigilance activities under supervision
• Perform ADR (Adverse Drug Reaction) reporting in accordance with hospital and regulatory guidelines
• Support prescription audits to ensure rational drug use and patient safety
• Review patient case sheets for identification and documentation of adverse events
• Participate in causality, severity, and preventability assessment of ADRs using standard scales
• Provide drug information support to healthcare professionals and nursing staff
• Assist in maintaining pharmacovigilance databases and safety records
• Support medication error reporting and risk minimization initiatives
• Participate in ward rounds for clinical data collection and patient monitoring (if required)
• Prepare summary reports and ensure accurate documentation for audits
• Coordinate with clinical teams for follow-up of reported cases
Qualification:
• Doctor of Pharmacy (Pharm.D)
• Master of Pharmacy (M.Pharm)
Skills:
• Basic knowledge of pharmacovigilance and ADR reporting
• Understanding of drug utilization review and prescription auditing
• Familiarity with causality assessment tools (WHO-UMC, Naranjo Scale)
• Strong documentation and data handling skills
• Good communication and interpersonal skills
• Patient safety-oriented approach
Duration:
• 6 Months (Internship)
