Job Description
This position involves providing pharmacy services for inpatients, day infusions, and discharge processes while ensuring compliance with clinical standards and the Pharmaceutical Benefits Scheme (PBS). The Hospital Pharmacist will work closely with hospital staff, patients, and carers, educating and advising on medicine use, health promotion, and disease prevention. The role includes both dispensing and clinical responsibilities, contributing to patient safety and effective healthcare delivery.
Key Features Of The Role
• Prepare Best Possible Medication History lists for hospital inpatients
• Conduct medication chart reviews and medication reconciliation
• Dispense and supply medications and pharmacy products
• Coordinate patient medication requirements for inpatient stay, day infusions, and discharge
• Develop and maintain professional relationships with key stakeholders
• Support patients, carers, hospital staff, and associated employees
• Educate and advise staff and customers on health promotion, disease prevention, and proper use of medicines
• Provide pharmacy services across 24-hour accident and emergency, general medical, specialist surgical care, anaesthetic, cardiac, obstetric, neonatal, chemotherapy, renal dialysis, rehabilitation, mental health, and stroke services
Responsibilities
• Ensure accurate and timely dispensing of medications in line with hospital protocols
• Conduct clinical review of medication charts and perform medication reconciliation for inpatients
• Collaborate with multidisciplinary teams to optimize patient outcomes
• Maintain knowledge of PBS requirements and hospital pharmacy policies
• Provide patient education and counseling regarding medication use and safety
• Monitor inventory and manage supply of pharmacy products
• Support hospital staff through training, advice, and consultation on medication-related queries
• Contribute to continuous improvement and service development initiatives within the pharmacy department
Required Qualifications
Educational Requirements
• Bachelor of Pharmacy (B.Pharm) or equivalent
• Current AHPRA registration as a pharmacist
Experience And Skills
• Previous dispensing or clinical experience in an Australian hospital or community pharmacy
• Strong knowledge of Pharmaceutical Benefits Scheme (PBS) processes
• Excellent communication, time management, and teamwork skills
• Ability to work independently with attention to detail and prioritization
