About the Role

Roles & Responsibilities
• Provide comprehensive clinical pharmacy services across the Local Health District
• Review, dispense, and manage medications in accordance with hospital policies
• Conduct medication reconciliation on admission and discharge
• Participate in multidisciplinary ward rounds and case discussions
• Monitor patient therapy outcomes and recommend evidence-based interventions
• Deliver patient counselling and medication education
• Provide drug information and clinical advice to medical and nursing staff
• Contribute to antimicrobial stewardship and medication safety programs
• Maintain accurate clinical documentation and medication records
• Support clinical audits, quality improvement, and risk management initiatives
• Supervise and support junior pharmacists or pharmacy students where required
• Ensure compliance with NSW Health policies and safety standards

Qualification
• Bachelor of Pharmacy or equivalent
• Current registration with Australian Health Practitioner Regulation Agency (AHPRA)

Experience
• Suitable for Pharmacist Grade 1 or Grade 2 classification
• Hospital or clinical pharmacy experience preferred

Skills
• Strong clinical and therapeutic knowledge
• Ability to work autonomously and within multidisciplinary teams
• Excellent communication and patient counselling skills
• Knowledge of medication safety and regulatory compliance
• Adaptability in rural and regional healthcare environments

Remuneration:
• Grade 1: $42.23 – $54.94 per hour
• Grade 2: $58.96 – $62.51 per hour
