Care Manager
About the Role
ROLE & RESPONSIBILITIES:
• Coordinate placement and deployment of home healthcare staff based on client requirements
• Conduct client onboarding, home visits, assessments, and periodic follow-ups
• Develop, review, and update individualized care plans in collaboration with clinicians
• Monitor care delivery and ensure adherence to quality standards
• Collect and analyze feedback from clients and families to improve service quality
• Build and maintain strong relationships with clients, caregivers, and families
• Provide guidance, communication, and support to field care workers
• Ensure timely and accurate payment collection as per company policies
• Maintain proper documentation, reports, and service records
QUALIFICATION:
• Physiotherapy
• Relevant certifications in healthcare or care management preferred
EXPERIENCE:
• Minimum 3 years of experience in healthcare or home care services
• Prior experience in team coordination or supervision preferred
SKILLS:
• Strong interpersonal and relationship management skills
• Excellent organizational and coordination abilities
• Ability to work independently and manage multiple clients
• Good communication and problem-solving skills
• Willingness to travel for home visits (two-wheeler preferred)
